Instructor/Independent Contractor Invoice Form

Instructors working as independent contractors for ISD 728 Community Education must submit an invoice for payment.

To assist you in creating an invoice, please complete the following online form and an invoice will be created.  A copy will be sent to you for your records, along with a copy automatically being sent to the Community Education Office.

Please note: In addition to sending an invoice, you need to submit a W-9 (Request for Taxpayer Identification Number and Certification) yearly.  You can find this form here; complete the form, save and then either attach it to an email and send to communityed@isd728.org or print and mail to Community Education, 1170 Main Street, Elk River, MN 55330.
